145 Pembroke Avenue is a 24-hour CMG supported living learning disability service in Enfield for adults with moderate to severe learning disabilities, communication impairments and additional health needs. Their goal is to deliver the best possible care to the people they support, including their health and wellbeing.
The staff team work closely with partners such as the GP, dietician, specialist therapists and other relevant external agencies. Family members and advocates also play a crucial part in decision making where appropriate.
To create a home environment, Pembroke Avenue actively encourage individuality and independence, by adopting a person-centred approach. All individuals have a person centred plan tailored to meet their needs. They understand that every moment has the potential to accomplish a goal no matter how small that goal is and all accomplishments are celebrated. Individuals are also supported to make a contribution to the community by sourcing voluntary or paid employment as well as having access to the amenities such as the cinema, gym, swimming pool, bowling alley and nearby shopping centres. Each tenant is supported to develop their own personalised schedule encompassing leisure interests, learning, enjoyment and activities of daily living.
