Our team at Beulah Crescent, our learning disability service in Croydon, provides community based support for adults aged 40+ with:
It is a specialist supported living service where individuals are supported to develop person centred plans and individualised health action plans that promote increased choice, control over their lives and independence.
Individuals are encouraged to be supportive of each other and develop their personal network of informal support. They also have the opportunity to become involved in a variety of educational, social and healthy living activities.
Care Management Group (CMG) has significant expertise in the provision of care and support for people with learning disabilities and particularly complex needs, including autistic spectrum conditions, mental health needs, challenging behaviour, profound and multiple learning disabilities (PMLD) and associated complex health needs. It has over 120 services in England and Wales.
